Key Responsibilities:

• Provide direct patient care in accordance with the care plan.

• Take vital signs, monitor patient conditions, and report changes to supervising RN/physician.

• Assist with activities of daily living (ADLs) such as bathing, feeding, and mobility.

• Administer medications and injections as per state regulations.

• Perform wound care, dressing changes, and catheter care.

• Collect lab samples and assist with diagnostic testing.

• Maintain accurate and timely patient records and documentation.

• Prepare and support patients for procedures and examinations.

• Operate basic medical equipment safely.

• Communicate patient needs, concerns, and changes to the healthcare team.

• Collaborate with RNs, physicians, and other healthcare professionals.

• Provide patient and family education on care, medications, and treatments.
